To report fraud or concerns to the Utah State Board of Education (USBE), you can call the Public Education Hotline at (801) 538-7813. You can also report concerns by: 



  • Filling out the online Report a Concern Form 

  • Emailing audit@schools.utah.gov 

  • Mailing a concern to:
    • Utah State Board of Education 

    • Internal Audit Department 

    • Attention: Debbie Davis 

    • PO Box 144200 

    • Salt Lake City, Utah 84114-4200 

  • Scheduling an in-person appointment by emailing audit@schools.utah.gov 

  • Visiting the Internal Audit Department in person at 250 E. 500 South, Salt Lake City
